BrightView’s Care Coordinators provide coordination, assessment, and treatment planning services to patients with substance use disorder (SUD). Across the critical spectrum of care, this position supports BrightView’s patients by facilitating access to needed medical, social, education and other community services essential to meeting basic human needs. Our Care Coordinators are a key member of our comprehensive care team and are focused on championing our patients’ ability to achieve a stable and productive lifestyle for lasting recovery.
